Manhattan’s Physician Group’s Registered Dietitians and Certified Diabetes Educators are now offering group patient appointments for diabetes self-management education and training.  Schedule an appointment today and get on the road to better management.

Monthly Group Diabetes Self-Management Flyer

Group patient appointments are taught using Healthy Interactions – Journey for Control Program (created in collaboration with the American Diabetes Association).  Over the course of four separate sessions, participants will actively engage with a diabetes educator in identifying and determining ways to achieve their short-term and long-term diabetes self-management goals.

Patients referred to diabetes self-management education and training will be required to schedule an individual appointment with the diabetes educator, as well as schedule four classes (offered once per month).

Weekday evening classes are available from 4:30-7pm* by appointment only, at the following Manhattan’s Physician Group locations:
